  3. Determine your caregivers by choosing from members of your family or friends, provided they meet program requirements and have been approved by the fiscal intermediary.
  4. To complete the application process, contact the fiscal intermediary you have chosen, who will help you start the application process. They will provide you with the necessary forms and information.
  5. Needs assessment: A healthcare worker, such as a nurse, will assess your needs and develop a care plan.
  6. Application confirmation: After the assessment and care plan development are completed, the fiscal intermediary will submit your application for approval to Medicaid.
  7. Hiring caregivers: After your application is approved, you can hire caregivers and start receiving necessary services.
